To set up an account with Precision Point Diagnostics, you must complete the Account & Billing Agreement form below and submit it. All testing must be ordered by a Physician or qualified Health Provider with an NPI number. Once the completed form is received by Precision Point and we process you into our system you will be given access to the Clinician Portal where you can order kits, view reports, and download test results. Being that our tests are specialized it can take up to 10 business days for results.

Effective July 1st, 2023, Precision Point Diagnostics will operate strictly on a 100% cash basis. As a result, we regret to inform you that we will no longer be able to accept commercial insurance, Medicare, or Medicaid for our functional diagnostic testing services.
This decision was not reached lightly, and we understand that it may impact your current billing and reimbursement processes. We assure you that this transition was made in the best interest of our laboratory and our ability to provide the highest quality of care to our patients. By eliminating the complexities and constraints associated with insurance billing, we can streamline our operations and focus on delivering exceptional diagnostic testing services with greater efficiency and accuracy.
